Abstract

Purpose of review To describe the current literature on clinical trials of probiotics for eczema and food allergy prevention in view of recent new approaches and long-term follow-ups.
Recent findings: Attempting allergy prevention by probiotic administration has been most successful when assessing atopic eczema, the most prevalent allergic disease at an early age. More than half of the published studies demonstrate a decrease in eczema prevalence until 2 years, whereas the remaining studies fail to show an effect. Effects have been most consistent with combined prenatal and direct postnatal supplementation of the infant and appear strain-specific, with Lactobacillus rhamnosus most often showing an effect. Prenatal-only and postnatal-only studies often fail to show effects. Recent long-time follow-ups have shown promising but not consistent results. A very recent follow-up of a large well conducted cohort shows that long-term effects of eczema prevention persists until age 4 and prevention of respiratory allergies might also be possible.
Summary: Prevention of eczema with probiotics seem to work until age 2 years and extended effects until 4 years have been shown in high-risk for allergy cohorts. Effects are strain-specific, with L. rhamnosus showing the most consistent effects especially when combining pre and postnatal administration.

There have been a few medical studies that have shown that probiotics can help reduce the incidence of food poisoning as well as reducing the duration of the illness and the severity of the symptoms.
Salmonella enterica, a pathogenic bacteria, is a very common cause of food poisoning. Animal studies have shown that administering probiotics has great potential to both reduce risk of Salmonella infection and its severity.  In 2007, researchers reported on a study they undertook whereby they gave healthy pigs a mix of five common probiotic bacteria (two strains of Lactobacillus murinus and one strain each of Lactobacillus salivarius subsp. salivarius,Lactobacillus pentosus, and Pediococcus pentosaceous) for six days.  They then infected the pigs with Salmonella enterica and monitored the exposed animals for the next 23 days.  Those pigs that treated with the probiotic cocktail showed reduced incidence, severity, and duration of diarrhea as well as a reduced number of Salmonella cells in their feces, as compared to the control group that were not given probiotics.  The study appears to show that giving probiotics prophylactically (to pigs at least) seems to reduce the risk of getting Salmonella, a potentially food-borne disease, as well as reducing the severity of the disease for those that did get it.  We look forward to human studies looking at this issue.
Note: When to Seek Immediate Medical Treatment

With relatively little downside (other than cost) and the potential to increase the enjoyment from a journey, it seems worthwhile to give strong consideration to taking probiotics before and during travel, particularly if the itinerary includes destinations such as India or Mexico, which are known hot-beds of Traveler’s Diarrhea!

Self-care: Probiotics for Food Poisoning

During a small study conducted at China’s North Sichuan Medical College and Hospital, researchers found that patients who suffered from traumatic brain injury also had compromised immune systems that were unable to fight infection.
Patients given probiotics in their feeding tube recovered faster and spent less time in intensive care than those patients treated with standard care.
Their bodies contained more infection-fighting T-cells, low levels of which can leave patients who are in fragile health susceptible to pneumonia and sepsis.
Study supervisor, Professor Jing-Ci Zhu, explained the benefit of an increase in T-cells. “Probiotic therapy reduced the number of infections occurring after seven days, reduced the number of different antibiotics needed to treat infections, and shortened the length of time the patients were required to stay in ICU.”
In the journal Proceedings of the National Academy of Sciences, researchers found that good gut bacteria sets the tone for how the rest of the body deals with infection and fights viruses.
Friendly bacteria helps the immune system respond faster and more effectively against a viral or bacterial threat.
The Cochrane Library journal published a similar study out of China that determined that probiotics benefits reduced the risk of upper respiratory tract infections by 12% in the 3,400 participants analyzed.
The British Journal of nutrition found that infants given Bifidobacterium lactis Bb-12 experienced less infections during their first year.

University College – Cork Professor John Cryan and his team found that probiotics showed that it lowered anxiety, depression and stress in mice. Specifically, Lactobacillus rhamnosus altered brain chemistry and lowered stress hormone production. Cryan’s team believes good gut bacteria communicate with the brain using the vagus nerve.
Professor Cryan explained, “We could develop therapies that can treat psychiatric disorders by targeting the gut. You could take a yoghurt with a probiotic in it instead of taking an antidepressant.
These findings highlight the important role of bacteria in the communication between the gut and the brain, and suggest that certain probiotic organisms may prove to be useful in stress-related disorders.”

There are some scientific studies that suggest consuming probiotics, especially Lactobacillus acidophilus, can help relieve the symptoms of lactose intolerance and allow you to digest milk and other dairy products more easily. But what happens when we look at traditional diets that were free of modern, processed conveniences? We find whole foods that are rich in both cholesterol and saturated fats. We also find no incidence of heart disease. (7)(8)

The latest medical literature reveals that there are many routes to heart disease, and that too many egg yolks is not necessarily one of them. In fact, for all the cholesterol in egg yolks, they are surprisingly protective. (9)(10)
If it is not cholesterol and saturated fats that lead to the buildup of atherosclerotic plaque, then what is the cause?
So far, the research points to several risk factors. A risk factor is something that increases a person's chance of getting a disease. Risk can be found in diet, lifestyle, or family history.
The risk factors for heart disease cover a wide spectrum of people and lifestyles. One thing that all heart disease risk factors have in common is oxidative stress. Oxidative stress happens on a cellular level. Excessive oxidative stress leads to the breakdown of cells and to tissue damage.
How does oxidative stress show up in the blood vessels? One way is in the form of atherosclerotic plaque. Atherosclerotic plaque is made up of:
  • Immune cells, which play an important role in the inflammatory process
  • Cell debris, which are pieces of cell from cells that have died
  • Crystalline cholesterol (11)
When tissue becomes damaged, it becomes inflamed. If this goes on for too long, you see an ongoing vicious cycle of inflammation, leading to tissue damage, which in turn causes more inflammation, and so on.
Inflammation leads to the development and build up of plaque. The most recent studies on heart disease have found that inflammation is more related to heart disease than elevated levels of cholesterol! (12)(13)
How Leaky Gut Can Increase the Risk for Heart Disease
When we talk about the relationship between the digestive system and heart disease, we are really talking about another repercussion of a leaky, or permeable, gut lining.
Once the lining of the gut wall becomes permeable, something called an endotoxin can get into the bloodstream. Endotoxins are found in the cell wall of many disease-causing bacteria. When these bacteria die, the endotoxin is released.
Three things to remember about endotoxins and their role in heart disease:
1. Our immune system reacts strongly to endotoxins. When detected, the immune system will send out the message to begin an inflammatory response.
2. Endotoxins, which are pieces of disease-causing bacteria, can become lodged in human tissue and remain there for years, continually igniting an immune response.
3. Endotoxins, being pro-inflammatory, generate oxidative damage.
Endotoxins have been found in atherosclerotic plaque. These endotoxins are gut-derived. What this means is that a leaky gut lining allowed endotoxins to escape into the bloodstream. Based on this information, you would think that those with chronically leaky guts, or full-blown celiac disease, would be at higher risk for heart disease. And you would be correct. Studies show an increase risk of heart disease in celiac patients. (14) (15)
But anyone can have a leaky gut, not just those people with celiac disease. These are just some of the ways the gut wall can become permeable, or leaky:
  • In times of stress, such as over-work or from lack of sleep
  • When we over-eat
  • From physical trauma
  • During a gut infection
  • After a bout of food poisoning

The relationship between infection and heart disease is so pronounced that one large trial even examined how effective an antibiotic may be in treating heart disease! IBS is a common functional disorder of the gut. A functional disorder means there is a problem with the function of a part of the body, but there is no abnormality in the structure. So, in IBS, the function of the gut is upset, but all parts of the gut look normal, even when looked at under a microscope. IBS causes various symptoms (listed below). Up to 1 in 5 people in the UK develop IBS at some stage in their life. IBS can affect anyone at any age, but it commonly first develops in young adults. IBS is slightly more common in women than it is in men.

Probiotics

Probiotics are nutritional supplements that contain good germs (bacteria). That is, bacteria that normally live in the gut and seem to be beneficial. Taking probiotics may increase the good bacteria in the gut which may help to ward off bad bacteria that may have some effect on causing IBS symptoms. You can buy probiotic capsules (various brands) from pharmacies. The dose is on the product label. You can also buy foods that contain probiotic bacteria. These include certain milk drinks, yoghurts, cheeses, frozen yoghurts, and ice creams. They may be labelled as 'probiotic', 'containing bacterial cultures' or 'containing live bacteria'.
There is some evidence that taking probiotics may help to ease symptoms in some people with IBS. At present, there are various bacteria that are used in probiotic products. Further research is needed to clarify the role of probiotics and which one or ones are most helpful. In the meantime, if you want to try probiotics, you should keep to the same brand of probiotic-containing product for at least four weeks to monitor the effect. Perhaps try a different probiotic for at least a further four weeks if the first one made no difference.

Probiotics may also prevent the uptake of lipopolysaccharides in the intestines, which may reduce the likelihood of liver irritation. This may mean that probiotic intake may drastically reduce the risk of liver disease in some people. The inhibition of pathogens by probiotic supplementation has been suggested as a means for fighting non-alcoholic fatty liver disease, or NAFLD. [2] In addition to fighting pathogens, probiotics may also modulate the overproduction of cytokines which leads to NAFLD, among other health concerns. Prebiotics may provide even further benefit by feeding the probiotics and supporting the proliferation within the gut microbiome.

Many links have been found between diet and cancer; research suggests that probiotics may be useful as part of an anti-cancer diet. Probiotics are live microorganisms that may suppress the growth of bacteria that transform procarcinogens into carcinogens. Procarcinogens are chemical substances that become carcinogenic after being altered by metabolic processes. A carcinogen is one of any number of substances involved in causing cancer. Research presented in 2009 at the 3rd International Immunonutrition Workshop in Spain demonstrated a link between milk fermented containing the probiotic strain Lactobacillus helveticus and delayed growth of breast tumors. The study found that fermented milks can inhibit tumor growth by reducing the inflammatory response in cases of hormone-dependent breast cancer models and induced colon cancer.

Probiotics are also known as lactobacillus acidophilus (L. acidophilus), Bifidobacterium bifidus and lactic acid bacteria. These are all healthy bacteria that are similar to the good bacteria that resides in the intestinal tract. Sometimes people do not have enough good bacteria in their system. Probiotic foods can increase levels of healthy bacteria. However, only live strains help. The ideal probiotics are those that contain large amounts of living microorganisms, such as multiple Bifido and Lactobacillus strains. At least three billion live bacteria per serving is best.


Probiotics and the immune system


Probiotics help to strengthen the immune system and prevent diseases. By improving gut health, probiotics boost the body's immune defenses, which in turn fight infections. The intestinal tract is home to between 70 and 80 percent of the body's immune cells. If the immune cells are functioning well the body is more resistant to infection. Hepatitis B, hepatitis C, the human papillomavirus (HPV) and Helicobacter pylori are the four primary infections linked to cancer. Combined, these four infections cause around 1.9 million cancer causes annually. Probiotics are particularly helpful in reducing vaginal and urinary infections.

When the gut has plenty of healthy bacteria and flora full of probiotics it can more easily digest food and expel waste. Toxins and waste can back up in the intestinal tract instead of moving through the colon and out of the body when there is not enough healthy bacteria. These toxins can then get absorbed back into the body through the bloodstream. Toxicity in the body can interfere with oxidation, which can cause stress to cells that are not getting enough oxygen. Stressed cells do not function as well, which leaves the body vulnerable to infection.


Probiotic supplements, foods and doses


Yogurt is the food most commonly associated with probiotics but there are several others. Unfermented and fermented milk contain probiotics. Kefir is another dairy product that is a probiotic food. Non-dairy sources include tempeh, miso and some soy drinks and juices.
